Case Management

What?

Case management is a crucial part of Catholic Community Services' mission. It is the main link between the agency and the children and families it serves. The agency provides support and services for children and families, including In-Home Safety and Non-Safety. It offers a range of placements, from traditional resource homes to specialized resource homes for children with special needs. These special needs include emotional, behavioral, autism, and medical needs. The agency also provides residential programs. Children served range in age from birth to 21.

Goals

Visitation

Case management services include regular home visits with children, conducted monthly or as needed on a weekly basis. Collaboration with families to achieve their established goals is central to the work of our case management team. When court-ordered, additional visits are held at the agency to ensure consistent support and engagement with families.

Reunification

The primary goal of case management is the safe reunification of children with their parents or family members. Reunification takes place when a child in a court-ordered placement is safely returned to their family. To support this process, case management staff regularly attend Family Court hearings, providing updates on the family’s progress, the agency’s efforts to meet court requirements, and advancement toward the goals outlined in the Single Case Plan.

Permanency

When reunification is not possible, the focus shifts toward achieving permanency through adoption or Permanent Legal Custody (PLC). The case management team includes a dedicated adoption unit that guides and supports children and families throughout the adoption process.

Collaboration

Case management collaborates closely with the agency’s Prevention & Intervention Team to provide comprehensive support for children and families. In addition, case management partners with external resources such as the DHS Nursing Department, Community Behavioral Health, the DHS Central Referral Unit, and various licensed private agencies that offer out-of-home care and specialized services for children.
